Requirements
- Direct SNF experience—preferably in Social Services, Discharge Planning, or Liaison roles, Excellent interpersonal and communication skills, Proven ability to work independently and solve problems under pressure, Experience presenting and promoting clinical programs to healthcare stakeholders, Ability to manage multiple facility relationships and meeting cadences, Clinical credentials (e.g., RN, LPN, LCSW, etc.) preferred, Experience with readmission prevention or transitional care coordination preferred, Sales or marketing exposure in a healthcare setting preferred
