Overview We're seeking a full-time Nurse Practitioner (NP) or Physician Assistant (PA) for onsite patient rounding in the post-acute and long-term care (PALTC) setting. This role supports smooth transitions from hospital to SNF and focuses on proactive, patient-centered care—identifying changes early, improving outcomes, and helping reduce avoidable hospital transfers/readmissions through consistent onsite clinical presence and strong interdisciplinary collaboration. - Experience as an NP or PA in primary care, internal medicine, hospital medicine, outpatient specialty, or SNF/post-acute care, Active California NP or PA license, Current CA DEA certificate, Strong ethics, reliability, and communication skills, Preferred: Geriatrics and/or SNF rounding experience
