Responsibilities
- Provide comprehensive primary care for geriatric patients with chronic and acute medical conditions
- Diagnose, treat, and manage illnesses using evidence-based practice
- Perform preventive care and wellness assessments
- Conduct patient evaluations in the clinic, PACE day center, and participants' homes as needed
- Collaborate closely with physicians and the interdisciplinary care team
- Coordinate specialty referrals and community healthcare services
Requirements
- Minimum 2 years of Nurse Practitioner experience
- Current and unrestricted Colorado Advanced Practice Nurse (APN) license
- Prescriptive authority (RXN) in Colorado
- National certification as an FNP, AGPCNP, or other appropriate Nurse Practitioner specialty
- Current DEA registration or eligibility to obtain
- Current CPR/BLS certification
