Responsibilities
- Conduct Annual Wellness Visits (approx. 60 minutes per patient)
- Perform COCM visits (30–40 minutes per patient)
- Provide episodic urgent care visits when needed
- Identify and close care gaps through comprehensive assessments and follow-ups
- Collaborate with patients primary care providers to ensure coordinated care
- Deliver evidence-based virtual care in a supportive team environment
Requirements
- Education: Graduate of an accredited <a href="https://medicalrecruiting.com/nurse-practitioner-recruiters" target="_blank" rel="noopener noreferrer">Nurse Practitioner program</a>
- Certification: ANCC or AANP board certification required (<a href="https://medicalrecruiting.com/nurse-practitioner-recruiters" target="_blank" rel="noopener noreferrer">FNP, ANP, or AGNP</a>)
- Licensure: Active Illinois NP license required; additional licenses in Tennessee, Michigan, or Texas highly preferred
- Experience: 1+ year post-graduate experience preferred, New graduates with board certification will be considered
- Experience with Annual Wellness Visits or urgent care strongly preferred
- Strong patient communication skills and ability to work independently in a virtual care setting
