Responsibilities
- Provide comprehensive primary care to a panel of adult patients (18+)
- Manage chronic diseases through a data-driven, team-based approach
- Utilize remote patient monitoring and other technologies to optimize patient care
- Collaborate with a multidisciplinary team of providers, medical assistants, dietitians, and other support staff
- Participate in a gradual onboarding process with a focus on quality over volume
Requirements
- Master of Science in Nursing (MSN) from an accredited program
- Current, unrestricted Nurse Practitioner license in the state of Indiana
- + years of experience as a Nurse Practitioner in a primary care setting is preferred
- New graduates with a strong RN background in ICU, cardiovascular, or other high-acuity settings are encouraged to apply
- Experience with remote patient monitoring and other healthcare technologies is a plus
- Strong interest in chronic disease management and a team-based approach to care

- Annual bonus potential up to $7,500, 401(k) with 3.5% company match after six months, 3 weeks PTO (4 weeks after 5 years), 3 paid holidays, United Healthcare medical insurance (50% employer-paid)
