Responsibilities
- Conduct comprehensive medical and functional assessments for patients in skilled nursing and rehabilitation settings
- Partner with physical and occupational therapists to develop individualized care and recovery plans
- Monitor patient progress and address any medical or functional barriers to rehabilitation
- Document patient encounters accurately using Doctor Now, integrated with PointClickCare
- Communicate effectively with nursing staff, therapists, and administrators to ensure coordinated care
- Participate in care meetings and initiatives to prevent rehospitalizations and improve patient outcomes
Requirements
