Responsibilities
- Conduct comprehensive assessments for patients in skilled nursing and rehabilitation facilities
- Collaborate with physical and occupational therapists to create individualized care plans
- Monitor patient progress and adjust treatment approaches to maximize recovery
- Communicate effectively with nursing staff, therapists, and administrators
- Document patient encounters accurately using Doctor Now, integrated with PointClickCare
- Participate in chronic-care management and initiatives to prevent hospital readmissions
Requirements
