Responsibilities
- Conduct comprehensive medical and functional assessments for patients in skilled-nursing and rehabilitation settings
- Collaborate with therapy teams to create and manage individualized care plans
- Monitor patient progress, remove barriers to recovery, and adjust care as needed
- Document encounters accurately using Doctor Now, integrated with PointClickCare
- Communicate effectively with nursing staff, attending physicians, and facility leadership
- Participate in chronic-care programs and readmission-prevention initiatives
Requirements

- Strong clinical judgment, organization, and communication skills
