Responsibilities
- Perform comprehensive assessments for patients in skilled nursing and post-acute care settings
- Collaborate with physical and occupational therapists to design and implement individualized rehabilitation plans
- Evaluate patient progress, identify barriers to recovery, and adjust care as needed
- Communicate with nursing staff, therapy teams, and facility administrators to coordinate care
- Document all encounters accurately using Doctor Now, integrated with PointClickCare
- Support patient success and prevent rehospitalizations through proactive management
Requirements
