Responsibilities
- Conduct comprehensive medical and functional assessments for patients in skilled-nursing and rehabilitation facilities
- Collaborate with physical and occupational therapy teams to design individualized care plans
- Monitor patient progress, identify barriers to recovery, and adjust treatment as needed
- Communicate with nursing staff, attending physicians, and facility administrators to coordinate care
- Document patient encounters accurately using Doctor Now, integrated with PointClickCare
- Participate in chronic-care management and initiatives that prevent rehospitalizations
Requirements
