Responsibilities
- Conduct medical and functional assessments for patients in skilled nursing and rehabilitation facilities
- Collaborate with physical and occupational therapists to develop individualized care plans
- Monitor patient progress and make clinical adjustments to promote recovery
- Communicate effectively with facility staff, therapists, and administrators
- Document patient encounters accurately using Doctor Now, integrated with PointClickCare
- Participate in care coordination and initiatives to reduce hospital readmissions
Requirements

- Strong clinical assessment and communication skills
- Experience or interest in Physical Medicine & Rehabilitation (PM&R) preferred
