Responsibilities
- Conduct comprehensive patient assessments focused on neuromusculoskeletal health and rehabilitation progress
- Work alongside physical and occupational therapists to design and adjust individualized care plans
- Monitor patient recovery, identify medical barriers, and support functional improvement
- Collaborate with facility staff, therapists, and administrators to ensure optimal outcomes
- Document patient encounters promptly using Doctor Now, integrated with PointClickCare
- Provide recommendations to the primary care team and participate in chronic care management initiatives
Requirements
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Illinois
- Strong interpersonal and communication skills
- Ability to work independently with confidence and professionalism
- Experience or interest in Physical Medicine & Rehabilitation (PM&R) preferred
