Responsibilities
- Conduct comprehensive assessments and manage treatment plans for patients in skilled nursing and rehabilitation facilities
- Collaborate with interdisciplinary teams—including physical and occupational therapists—to optimize functional recovery
- Evaluate progress, remove barriers to rehabilitation, and coordinate with facility leadership to ensure quality outcomes
- Document encounters and care plans in Doctor Now, integrated with PointClickCare
- Participate in interdisciplinary meetings and communicate effectively with nursing and therapy staff
- Support initiatives in chronic care management and hospital readmission prevention
