Responsibilities
- Conduct thorough medical and functional assessments for patients in skilled nursing and rehabilitation facilities
- Collaborate with interdisciplinary teams, including physical and occupational therapists, to design effective, individualized care plans
- Monitor and document patient progress using Doctor Now, integrated with PointClickCare
- Participate in clinical meetings and coordinate with facility staff to ensure seamless patient care
- Identify and address barriers to rehabilitation to optimize outcomes and recovery
- Support chronic care management and readmission prevention programs
