Responsibilities
- Conduct comprehensive medical and functional assessments for patients in skilled nursing and rehabilitation facilities
- Collaborate with interdisciplinary teams—including physicians, physical therapists, and occupational therapists—to design and implement effective treatment plans
- Monitor and document patient progress using Doctor Now, integrated with PointClickCare
- Participate in clinical meetings, coordinate with nursing staff, and address rehabilitation barriers to optimize outcomes
- Provide recommendations to the primary care and facility teams to support patient recovery and long-term success
- Support chronic care management and hospital readmission prevention programs
Requirements
