Responsibilities
- Conduct comprehensive medical and functional assessments of patients in skilled nursing and rehabilitation facilities
- Collaborate with an interdisciplinary team—including physicians, physical therapists, and occupational therapists—to develop and monitor individualized care plans
- Manage and evaluate patient progress, addressing medical barriers to recovery and optimizing outcomes
- Participate in daily clinical rounds and coordinate with nursing staff and facility administrators to ensure quality care delivery
- Document visits and care plans using the Doctor Now EHR, integrated with PointClickCare
- Provide ongoing communication with the primary care team, contributing to chronic care management and readmission prevention programs
Requirements
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Massachusetts
- Strong interpersonal, communication, and clinical assessment skills
- Experience or interest in Physical Medicine & Rehabilitation (PM&R) preferred
- Ability to work independently while collaborating with a multidisciplinary team
