Responsibilities
- Perform comprehensive assessments for patients in skilled nursing and rehabilitation settings
- Collaborate with physical and occupational therapists to develop and manage individualized treatment plans
- Evaluate patient progress, remove barriers to recovery, and coordinate care across disciplines
- Document encounters and care plans promptly using Doctor Now, integrated with PointClickCare
- Attend interdisciplinary meetings and communicate effectively with nursing staff and facility leadership
- Participate in chronic care and rehospitalization prevention initiatives to enhance patient outcomes
Requirements
- Licensed <a href="https://medicalrecruiting.com/nurse-practitioner-recruiters" target="_blank" rel="noopener noreferrer">Nurse Practitioner (NP)</a> or <a href="https://medicalrecruiting.com/physician-assistant-recruiters" target="_blank" rel="noopener noreferrer">Physician Assistant (PA)</a> in Illinois or Missouri (dual licensure preferred or willingness to obtain)
- Strong interpersonal and clinical skills
- Experience or interest in Physical Medicine & Rehabilitation (PM&R) preferred
- Ability to work independently and within an interdisciplinary team
