Responsibilities
- Conduct comprehensive patient assessments and manage care plans focused on neuromusculoskeletal health and rehabilitation
- Collaborate with interdisciplinary teams to support individualized treatment goals
- Track patient progress, adjust care plans, and provide ongoing updates to attending physicians and facility teams
- Participate in care conferences and communicate with nursing and therapy staff
- Document encounters using Doctor Now, integrated with PointClickCare, ensuring accuracy and timeliness
- Contribute to chronic care management and rehospitalization prevention initiatives
Requirements
