Responsibilities
- Conduct detailed assessments of patients in skilled nursing and rehabilitation settings
- Collaborate with physical and occupational therapists to create and monitor individualized rehabilitation plans
- Manage patient progress, address barriers to recovery, and coordinate with facility teams
- Document visits using Doctor Now, integrated with PointClickCare, ensuring accurate and timely reporting
- Attend interdisciplinary team meetings to enhance patient outcomes and communication
- Provide leadership in chronic care management and readmission prevention programs
Requirements
