Responsibilities
- Conduct comprehensive patient assessments, focusing on neuromusculoskeletal health and rehabilitation outcomes
- Develop and adjust individualized treatment plans in collaboration with interdisciplinary teams, including physical and occupational therapists
- Evaluate patient progress, identify barriers to recovery, and communicate findings effectively with facility staff and attending providers
- Attend clinical meetings, coordinate with nursing staff, and promote continuity of care
- Document encounters and care plans promptly using Doctor Now, integrated with PointClickCare
- Support chronic care management and prevent hospital readmissions through proactive patient monitoring
Requirements
- Licensed <a href="https://medicalrecruiting.com/nurse-practitioner-recruiters" target="_blank" rel="noopener noreferrer">Nurse Practitioner (NP)</a> or <a href="https://medicalrecruiting.com/physician-assistant-recruiters" target="_blank" rel="noopener noreferrer">Physician Assistant (PA)</a> in Michigan or Ohio (dual-state license preferred but not required)
- Strong communication and clinical assessment skills
- Ability to work independently while collaborating within a team-based environment
- Experience or interest in Physical Medicine & Rehabilitation (PM&R) preferred
