Responsibilities
- Conduct patient assessments in skilled nursing and rehabilitation facilities, focusing on neuromusculoskeletal health and functional recovery
- Collaborate with physical and occupational therapists to develop and manage individualized care plans
- Monitor patient progress, document visits using Doctor Now integrated with PointClickCare, and identify opportunities to improve outcomes
- Communicate effectively with nursing staff, attending providers, and facility administrators to coordinate care
- Participate in interdisciplinary care meetings and provide recommendations that support long-term rehabilitation success
Requirements
