Responsibilities
- Conduct comprehensive assessments for patients in skilled nursing and rehabilitation settings
- Collaborate with therapy teams to design and monitor individualized treatment plans

- Communicate effectively with facility staff, attending physicians, and administrators

- Participate in chronic care management and care coordination initiatives as needed
